Rehabilitation at Peartree House
The Peartree rehabilitation team includes a number of allied health professions, dedicated to ensuring that the client’s rehabilitation potential is developed and maximised.
Initial assessment is the foundation for the person centred programmes which are evaluated and updated at the weekly Multi disciplinary meeting. Regular formal reviews are attended by the multi disciplinary team. These ensure that the progress of this rehabilitation is monitored by all the multiple services, including the client, care manager, relatives, social services and the funders.
Relatives are encouraged to participation in family meetings, formal reviews, the development of care plans and therapy sessions. A team approach enables families to get to know all members of staff and a family support group meets regularly, chaired by a relative.
